Альтернативні методи фізичної терапії хворих після лівостороннього геморагічного інсульту на ранньому відновному етапі

Abstract

Досліджувались поширеність лівостороннього геморагічного інсульту, його симптоми, ознаки, наслідки, класифікація та причини виникнення, методи діагностики. Показано, що поширеність геморагічного інсульту щорічно становить близько 15% всіх інсультів, з них: 10% внутрішньомозкові крововиливи, 5% субарахноїдальні крововиливи. Важливо, що більше третини (35,5%) мозкових інсультів зустрічаються в осіб працездатного віку. Основна частина роботи присвячена дослідженню впливу альтернативних методів фізичної терапії, що базуються на фізіологічному тренуванні мозку, яке відновлює його правильну роботу, на механізмі нейропластичності. Визначено, що використання альтернативних методів фізичної терапії після перенесеного лівостороннього геморагічного інсульту значно покращує якість життя та стан пацієнта: прискорює часткове (інколи повне) відновлення мовних та вестибулярних функцій, усунення дисфагії, сприяє розширенню рухових функцій паретичних кінцівок. Показано, що максимальне відновлення порушених функцій досягається при поєднанні класичних засобів реабілітації (кінезітерапія, масаж, фізіотерапія, механотерапія, ерготерапія) та сучасних альтернативних методів фізичної терапії.

According to the scientific and methodological literature, the prevalence of left-sided hemorrhagic stroke, its symptoms, signs, consequences, classification and causes, methods of diagnosis were studied. According to official statistics, 100,000–110,000 strokes occur in Ukraine every year (more than a third of them are of working age), 30–40% of stroke patients die within the first 30 days and up to 50% within a year of the onset of the disease; 20–40% of surviving patients become dependent on outside care (12.5% of primary disability) and only about 10% return to full life. Importantly, more than a third (35.5%) of strokes occur in people of working age. Rehabilitation measures in hemorrhagic left-sided stroke should be aimed primarily at preventing and inhibiting neurological manifestations, which depends on the restoration of neurons that remain morphologically intact, to suppress the mechanisms of creating a zone of functional disconnection (asinapsia) of neurons. It is equally important to prevent the development of atrophic processes in the muscles, their atony, to maintain and develop normal static and dynamic stereotypes. Today, alternative methods of physical therapy are widely used, based on physiological training of the brain, which restores its proper functioning, on the mechanism of neuroplasticity. Special attention in the early recovery period deserves methods based on the mechanisms of neuroplasticity the ability of various parts of the central nervous system to reorganize due to structural changes in the substance of the brain. It is shown that the maximum restoration of impaired functions is achieved by combining classical means of rehabilitation (kinesitherapy, massage, physiotherapy, mechanotherapy, occupational therapy) and modern alternative methods of physical therapy.
