This disease affects mainly young people in the second and third decades. Previously, it was believed that the incidence of the disease in women is only 10% of that in men. Now we know that the disease in female patients is often only slowly leading to ossification and therefore, previously, was not generally recognized as a disease by Bechterev. This disease is diagnosed in women almost as often as in men. Today it is estimated that more than 1% of Central European residents suffer from this disease in the presence or absence of a definitive diagnosis and that 0.1-2.0% of the disease is expressed in such a way that they need to be treated and finally diagnosed with ankylosing spondylitis (AS) "Bechterev's disease". In other parts of the globe, there is a different frequency of diseases in places, the reason for this is not in the change of climate or lifestyle, but mainly in the generic differences. This is evidenced, for example, by the fact that this disease is found both in the indigenous population of Africa and among the black population of America less than in the white population of Europe and America. Also, the inhabitants of eastern Asia, this disease is relatively rare, and, conversely, the Indians are much more common than ours. The cause of Bechterev's illness remains unclear. It is known that this should be about the incorrect functioning of the protective system of the organism, which leads to the fact that the protective system is confronted not only with the struggle against penetrating pathogens, but also against the cells of its own body.
