Зміни функції зовнішнього дихання у хворих на хронічне обструктивне захворювання легень при застосуванні фізичної реабілітації

Abstract

Мета – вивчити ефективність впливу запропонованої методики фізичної реабілітації на функцію зовнішнього дихання (ФЗД) хворих на хронічне обструктивне захворювання легень (ХОЗЛ). Обстежено 416 хворих на ХОЗЛ 1, 2 і 3 ступенів тяжкості, чоловіків було 216 (51,92%), жінок – 200 (48,08%), середній вік яких склав 56,82±0,84 років. В основній групі хворим до стандартного лікування додатково призначалася розроблена програма фізичної реабілітації. Застосування фізичної реабілітації призводить до суттєвого покращення ФЗД хворих на ХОЗЛ: при 1 ступені тяжкості об’єм форсованого видиху за 1 с в основній групі зростає з 81,14±2,15 до 90,24±2,32% від належної величини (НВ) (р 0,001), в контрольній групі – з 80,95±1,91 до 84,75±1,66% від НВ (р 0,001); при 2 ступені тяжкості: в основній групі – з 63,39±1,68 до 71,35±1,99% від НВ (р 0,001), в контрольній групі – з 62,84±1,40 до 67,74±1,77% від НВ (р 0,001); при 3 ступені тяжкості: в основній групі – з 42,06±1,56 до 51,20±1,67% від НВ (р 0,001), в контрольній групі – з 40,81±1,59 до 45,54±1,57% від НВ (р 0,01). У хворих основної групи показники ФЗД при 1 ступені тяжкості в середньому в 2,2 рази, при 2 ступені тяжкості – в 1,9 раз, при 3 ступені тяжкості ХОЗЛ – в 2,1 рази перевищують такі в порівнянні зі стандартним лікуванням. За результатами застосування запропонованої фізичної реабілітації досягнутий стан ФЗД хворих на ХОЗЛ при любому ступені тяжкості за деякими показниками суттєво переважає такий при стандартному лікуванні. Застосування запропонованої програми фізичної реабілітації у хворих на ХОЗЛ призводить до суттєвого покращення ФЗД при всіх ступенях тяжкості захворювання.

Chronic obstructive pulmonary disease (COPD) in the medical, social and economic terms is one of the main causes of morbidity and mortality worldwide; people suffer from this disease for years and prematurely die of it or its complications. The purpose – to study the effectiveness of the proposed method of physical rehabilitation on the external respiration function (ERF) of patients with COPD. 416 patients with COPD were examined for 1, 2 and 3 degrees of severity, men were 216 (51.92%), women – 200 (48.08%), whose average age was 56.82±0.84 years. In the main group of patients the standard treatment was additionally intended to develop a program of physical rehabilitation. The use of physical rehabilitation leads to a significant improvement in the ERF of patients with COPD: at 1 degree of severity, the forced expiratory volume in the main group increases from 81.14±2.15 to 90.24±2.32% of the proper value (PV) (p<0.001), in the control group – from 80.95±1.91 to 84.75±1.66% of the PV (p<0.001); at 2 degrees of severity: in the main group – from 63.39±1.68 to 71.35±1.99% of the PV (p<0.001), in the control group – from 62.84±1.40 to 67.74±1.77% of PV (p<0.001); at 3 degrees of severity: in the main group – from 42,06±1,56 to 51,20±1,67% of PV (p<0,001), in the control group – from 40,81±1,59 to 45,54±1,57% of PV (p<0,01). In patients with the main group, ERF values at 1 degree of severity averaged 2.2 times, with 2 degree of severity – 1.9 times, with 3 degree of COPD – 2.1 times higher than the standard treatment. According to the results of the proposed physical rehabilitation, the achieved state of ERF in patients with COPD at any degree of severity by some indicators is significantly superior to the standard treatment. The application of the proposed program of physical rehabilitation in patients with COPD leads to significant improvement of ERF at all degrees of disease severity.
